We know that every extra form, required field, and administrative step can become another barrier between a family and the support they need. So we deliberately keep our referral process simple.
If you’re referring a patient to Pinwheel Clinic, just send us an EMR-generated referral or letter with the relevant information you already have. Please include the patient’s demographic and contact information so our team can reach out directly.
There is no Pinwheel-specific referral form to complete and no need to duplicate information that is already in the medical record.
We accept referrals from any healthcare, education, or community professional involved in a child or family’s care.
Families can also self-refer directly to Pinwheel Clinic. A professional referral is not required.
